Long Term Conditions (LTC) are health conditions that can impact on a person’s life, and may require ongoing care and support. Long Term Conditions include conditions such as Asthma, CHD, Diabetes and Hypertension.
We invite all patients who have a LTC to attend for an annual review each year to check on their condition and ensure it is being managed in the best way possible.
