Adult Referrals
You have been directed to this page because you have advised us that you believe you or someone you look after has ADHD and/or Autism. This page will give you the step-by-step process for being referred for further assessment for these conditions.
You can use this checklist to ensure you are ready to be referred.
All of the attachments on this page can be printed off for you at the practice. Please ask for the ADHD/Autism Pack at reception if you would like a paper copy.
Please ensure you read all of the information on this page carefully.
Step One – You will need to fill out the relevant questionnaire for the referral(s) you are wishing to pursue. You can download the questionnaire(s) here or we can print these off for you, if you would like a printed copy please collect from reception:

AUTISM ADULTÂ Â Â Â Â
If you intend to be referred for both ADHD and autism assessments, you must complete both of the relevant questionnaires as these are two separate referrals.

Step Two – You will now need to check your eligibility for referral.
ADHD ADULT – You must have a score of 4 or more in Part A
AUTISM ADULT – You must have a score of 6 or more to be eligible for referral
If you do not reach these scores but feel you need further help, we cannot refer you for an assessment, but you can book a routine appointment with a GP to discuss your symptoms.

Step Three – you will now need to decide where you wish to be referred to.
The practice can refer you to the local NHS provider or you can visit the link below to see what other providers are available. The practice is not able to choose for you. If you are being referred for both autism and ADHD assessments, you will need to let us know which provider you want to be referred to for both.
When selecting a Right to Choose provider, it is vital to ensure that:
- You meet the referral criteria for that provider.
- If you wish to consider taking medication, please ensure you chosen clinic starts and titrates medication should it be offered. Shared prescribing is assessed on a case by case basis.
- You understand that waiting times can change, we are unable to keep up to date with the changes.
- Most providers have different referral forms. Cofton Medical Centre use one standardised form for these referrals. This is line with our GMS contractual responsibility, we have also sought BMA and LMC guidance.

Step Four – Now you have chosen your provider(s), you need to complete the referral form(s):
We can print these off for you if needed, please visit the practice obtain copies.

Step Five – Return the questionnaire(s), referral form(s) and any other supporting evidence to the practice and we will process these for you within 10 working days.
Please be assured, that we fully support your journey to assessment and diagnosis and appreciate your cooperation with the process to ensure that this can be processed as quickly as possible.
Seeking assessment and diagnosis is a very personal journey, as a result we will not follow up with patients who have not returned their forms as we appreciate you will continue with this process when you are ready.

If requesting referrals for both ADHD and autism assessments, you must complete both of the relevant questionnaires.

Step Two – You will now need fill out the referral form, it is important to liaise with the child’s education setting for this step
Child Neurodevelopmental pathway referral form
Your child’s education setting will play an integral part in providing supporting evidence for your child’s referral(s). Please ensure they have completed part 3 of the referral form. The education setting will know your child more than your GP will. If your child is homeschooled, please fill out the questions as best you can.

- Your child meets the referral criteria, please note if your child is under the age of 5 they will not be able to be referred through the right to choose pathway. Please speak to us or your health visitor about local support or early development services while you wait.
- If wishing to consider taking medication, should it be offered, please check if the provider will prescribe this. We do not prescribe these types of medication for children.
- You are aware that waiting times can change, we are unable to keep up to date with the changes
