How to Register
The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a new patient health check with one of our nurses.
Medical treatment is available from the date of registration. Please contact reception for further information.
If you would like to register with Cofton Medical Centre you can do so by using the online registration form. Alternatively, you can visit the surgery and collect a registration form. Whether you are completing the paper form or the online form you will need to complete a separate form for each person you are registering with the practice.
Please try to complete all forms as much as possible, missing forms or information may delay your registration.

The National Care Record Service (NCRS)
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
